GREAT PAY – $1,500+, plus you may have the opportunity to meet and be on camera with a top celebrity and/or company spokesperson for the shoot.

PEOPLE WITH HIGH-RISK CHOLESTEROL LEVELS WHO HAVE HAD A HEART ATTACK OR STROKE
CASTING COMMERCIAL TO HELP BRING AWARENESS
We are searching for people with HIGH-RISK CHOLESTEROL LEVELS (age 50+) who have ALREADY had a heart attack or stroke and who are in the greater Los Angeles area to tell their story in a commercial that will help bring awareness.
GREAT PAY – $1,500+, plus you may have the opportunity to meet and be on camera with a top celebrity and/or company spokesperson for the shoot.
Patient Requirements:
– Patient must have h high-risk cholesterol levels
– Patient MUST have already had either a heart attack or stroke
– Patient must be comfortable appearing on-camera with celebrity and/or HCP spokesperson
– Age 50+
Open to male and female, all ethnicities.
Patient Spokesperson Responsibilities:
– Appear in video content that will be used in a digital/online video housed on a commercial campaign landing page
It’s a PLUS if any of the following situations apply, so please let us know:
• Family history of cardiovascular disease
• Suffer from peripheral artery disease (PAD)
• Diagnosed with familial hypercholesterolemia (FH)
• Let us know if you have any media or public speaking experience!
• We want to hear if you have a compelling personal story or backstory
• Do you have a patient/caregiver story (e.g., husband and wife, etc.)
Casting: Now, submit no later than Thursday, December 13th, 2018
Shoot: December 20th, 2018, in Los Angeles
